CHIEF COMPLAINT
With whole body swelling since one week and diabetic
HISTORY OF PRESENT ILLNESS
Patient is having whole body swelling since one week and he doesn't have any kind of pain
HISTORY OF PAST ILLNESS
Diabetic from past 5 to 6 yrs
Hypertension 
MEDICAL HISTORY
Metformin-500mg
Glimipiride-1 mg
Talmistartan-40 mg
Vitamin D3
FAMILY HISTORY
No similar complaints
PERSONAL HISTORY
Diet- mixed diet
Appetite-normal
Sleep-normal
Bowel / bladder-normal
GENERAL EXAMINATION
Conscious, cooperative
Patient was built normaland well
Pallor-present
Icterus- absent
Cyanosis- absent
Clubbing-absent
Lymphadenopathy- absent
Edema- Ansarca, pitting edema
