                           CASE HISTORY
Patient detail
A 45 year's old male ,of occupation- business resident of miryalaguda Presented with
CHIEF COMPLAINT
   Ulcer on his left foot which had fracture 1 year back
Cp moHISTORY OF PRESENT ILLNESS
Patient was apperently asymptomatic  one year back layer he met with an accident and got fracture on his left leg he went to nearby hospital and got orthopedic surgery and also he got a wound at the same time got dressing for that wound, after few months he observed that the wound is not healing and skin over wound is not forming properly  so by the suggestion of  other doctor he visited hospital 
Pain - dull ache
HISTORY OF PAST ILLNESS
Had orthopedic surgery one year back and took medication for it
Non diabetic
No hypertension
Having poliomyelitis since childhood
PERSONAL HISTORY
Diet- vegetarian
Sleep-adequate
Bowel/Bladder- regular
Apettite- normal
FAMILY HISTORY
No similar complaints
GENERAL EXAMINATION
Normal ,well built, conscious ,cooperative
Nourishment-good
Pallor- absent
Icterus-absent
Cyanosis-absent
Clubbing-absent
Lymadenopathy- absent
Edema - absent
                        
     ----PROVISIONAL DIAGNOSIS-----
          May be a bacterial infection


Q/A
1. Does his past history of poliomyelitis may effect the present situation?
2.what may be the exact reason for not developing of skin?
